Deadline for paper registration is postmarked Monday, October 7, 2013. You may register online at www.nphchq.org until Monday, October 14, 2013. If you have difficulty with online registration, print out the error page (s), keep them for your records, and contact NPHC Headquarters.

Please complete forms in their entirety to help us better, and more efficiently, serve you. Be sure to type or print legibly all requested information. An e-mail address must be included. Check the appropriate registration category. Incomplete/incorrect registrations will not be processed and will be returned.
Payment
DO NOT SEND CASH. Registration payment may be made by money order, council check, certified check or college/university issued check. Personalwill not be accepted. For two or more persons, please submit one form of payment per council, with both council and individual registration forms enclosed. Make college/university issued checks, money orders, or cashier’s checks payable to
National Pan-Hellenic Council, Inc. or NPHC, Inc., and mail to

Registration will be confirmed by electronically by Monday, October 17, 2013.

Disclaimer & Refund Policy
There is NO ONSITE Registration for this conference.
Registration fees, paid in advance, are refundable upon receipt of written notification of cancellation to NPHC National Headquarters and a legitimately approved reason for cancellation (see Refund Schedule below). The request should be in the form of a letter, or electronic message, stating the reason for cancellation with verification signatures of the council president and advisor, if applicable. All contact information of the petitioner and advisor (if applicable) should be included.
Refund Schedule: Requests postmarked on or before September 1, 2013 will be charged a 50% cancellation fee; requests after September 15, 2013 will be charged a 75% cancellation fee. Requests postmarked after September 16, 2013 will not be honored. Tickets for subscription events are not refundable. Refund checks will be issued approximately 120 days after conference.
